An Audit Report Is a Risk Map, Not a Green Light
Founders keep treating audit reports like launch certificates. They are narrower and more useful than that: a snapshot of scope, assumptions, and residual risk.
That distinction matters because capital formation, partner diligence, token launches, and protocol deployments still get framed around a simple question: "Has it been audited?" Serious teams should ask a different one: what exactly was reviewed, under which assumptions, on which commit, and what still remained unresolved when the report was published?
Establish the problem with technical depth
If you want a concrete example of why this matters, look at Euler. In its post-incident writeup, Euler says the protocol was exploited on March 13, 2023 for roughly $197 million. The team later traced the root cause to a single missing line of code in an obscure donateToReserves path: a health check was missing, which let the attacker move an account into an unhealthy state and then self-liquidate for profit. The painful part is not just the number. It is that the vulnerable path came from a patch added to fix an earlier bug and the change had been audited.
That is how founders misread audit reports. They see "audited" and hear "safe." But an audit is not a promise about your company, your treasury, or the next upgrade. It is an expert assessment of a bounded code state and a set of assumptions at a point in time.
OpenZeppelin's public audit documentation is useful here because it states the quiet part out loud. The executive summary sits next to the scope, timeline, auditors, privileged roles, trust assumptions, issue severities, issue statuses, recommendations, and fix-review trail. In other words, the report is not only telling you what was found. It is telling you how to interpret the boundaries of what was reviewed.
A recent OpenZeppelin report for The Compact makes this explicit. The summary records 31 total issues, with 25 resolved and 2 partially resolved, and the scope names an exact repository commit. It even breaks the work into phases, including a later diff audit between a base commit and a head commit. That is what a serious audit looks like: versioned scope, explicit findings, and visible residual work. If a founder skips those sections and reads only the summary page or the tweet thread, they are not consuming diligence. They are consuming marketing.
For investors and boards, this is not a technical footnote. It changes how risk should be underwritten. A protocol can have a credible audit and still carry unresolved assumptions around admin power, upgrade rights, oracle dependencies, or offchain actors. The report may even say so plainly. If no one on the business side reads that section, the company starts pricing itself off a badge instead of a threat model.
The mechanism, the mistake, the misunderstanding
The first thing to read in an audit report is scope. Not the severity table. Not the executive summary. Scope.
Which repository was reviewed? Which commit? Which files? Was it a full review, an incremental review, or a diff audit after changes? The Compact report, again, is clear: OpenZeppelin audited a specific commit, then later performed a diff audit between two named commits. That one detail tells you something critical. Audit coverage is not abstract. It is attached to versioned code.
The second thing to read is the overview. OpenZeppelin's audit docs explain that this section often spells out architecture, privileged roles, and trust assumptions. That is where the report stops behaving like a bug list and starts behaving like a business document. If the protocol depends on multisig operators, offchain relayers, allocator behavior, oracle freshness, or a privileged pauser, that is not background color. That is part of the attack surface.
The third thing to read is issue status, not just severity. Many non-technical readers skim for "no criticals" and stop. That is a rookie mistake. OpenZeppelin's documentation lists statuses such as Unresolved, Responded, Resolved, Partially Resolved, Acknowledged Not Resolved, and Acknowledged Will Resolve. Those labels matter because they tell you whether a risk was actually fixed, partially mitigated, deferred, or accepted.
The fourth thing to read is the fix-review trail. The OpenZeppelin audit readiness guide lays out the expected flow clearly: the audit team reviews code, reports findings, the client fixes issues, and the audit team may review those fixes. That means an audit is not a single PDF event. It is a workflow. If the report you are reading does not make it obvious which findings were re-reviewed after remediation, you should assume the gap matters.
This is also where the biggest misunderstanding lives. Founders tend to treat an audit as if it answers, "Will we get hacked?" It does not. It answers something narrower and more useful: "What did qualified reviewers learn about this scoped system, on this code state, with these assumptions, and what should be fixed or monitored next?"
That narrower framing is not a weakness. It is what makes the report usable. But only if you read it like an operator instead of a marketer.
What good looks like
Good audit consumption is disciplined. It does not require the founder to become a smart contract auditor overnight, but it does require asking the right questions.

Match the report to the exact code that is going live. If the report covers commit

Read the trust assumptions and privileged roles before reading the conclusion. If one multisig, relayer, oracle, or emergency admin can freeze funds or alter execution, that belongs in diligence and board-level risk discussion.

Separate "resolved" from "accepted." A report with no criticals can still contain medium or low issues that become catastrophic in combination, and partially resolved findings deserve real follow-up.

Ask how the team tests behavior that the audit could not prove. OpenZeppelin's readiness guide is blunt: code should be well-tested, edge cases matter, local fork testing matters, and more advanced projects should incorporate fuzzing and property-based testing. An audit without strong tests behind it is thinner than it looks.

Treat upgrades as new security events. Euler's story is the warning label. A patch for one bug introduced the path for a much larger exploit. Every upgrade, adapter, governance change, and parameterized rollout needs fresh review proportional to the change.
For CTOs and lead engineers, the operational translation is straightforward. The audit report should become an internal security backlog, a monitoring plan, and a pre-deploy checklist. Findings get mapped to commits. Assumptions get mapped to tests and runtime alerts. Privileged roles get mapped to operational controls. If that translation never happens, the PDF never becomes a control.
For founders and VCs, the practical question is even simpler: after reading this report, do I know what could still break the system? If the answer is no, you have not finished reading.
